Unusual cases: Case 2

This is indeed an uncommon type of restoration but not an unusual problem. This is an older patient that had taken reasonably good care of herself over the years. But she had significant orthodontic issues that were never addressed years before and was restored in this bad position. To put it simply, she was way over-closed. Her jaws had become too close together as a result of tooth loss and other boney changes over the years. After many years of good service out of her restorations, some critical teeth broke down and she was not up to having major restorative work or orthodontics done at this point in her life. But she did need to still be able to go out in public and have something to function with. The only thing I could do for her to meet her needs and desire for minimal treatment was a type of over-denture. Usually, we think of over-dentures as ones which are placed over tooth roots, implants or metal castings of some sort. In this case I needed to be able to open her bite quite significantly just to make room for the replacement teeth. I had made her a temporary appliance first just to see if she would tolerate the required degree of bite opening. She did fine and in the end we accomplished our goal. She may not use the appliance when she’s home because she’s able to function with the few remaining teeth she has but she can now go out in public without feeling self-conscious about her smile. Self-esteem is just as important for a ninety year old as it is for people many decades younger
