Full Mouth Rehabilitation: Case 3

This young/middle age guy came in seeking rehabilitation of his dentition after years of neglect. He actually had almost no decay, but he had lost several teeth, had periodontal disease, spacing/orthodontic and bite issues. The most apparent thing to the casual observer is the extreme wear of his front teeth. After a complete diagnostic workup periodontal treatment followed by orthodontic therapy was recommended to re-establish proper spacing and open his collapsed bite. This would be followed by implant therapy to replace missing teeth and lastly restoration of his broken down teeth and satisfy his cosmetic needs. Lastly, a night guard was recommended to protect his newly restored dentition. However as is often the case for adults he was not willing to commit the time required for orthodontic and implant therapy and opting for more immediate gratification in the form of whiter straighter looking teeth with crown and bridge along with four lower anterior veneers. He felt he was functioning well enough with the number of teeth he had and elected to retain some teeth that will likely be problematic as the years go by, choosing to deal with them in the future. Sometimes the we as dentists have to be willing to compromise ideal treatment as long as we are doing no harm, improve our patient’s health and satisfy their needs whether they are functional, cosmetic or both. This case was driven primarily by the patient’s cosmetic desires even though functionally he also had significant problems. We were both very happy with the final outcome all things considered.
